To All University Students
This e-mail is being sent to all students as a reminder of the Student Health Insurance Enrollment/Change Period, which runs 5/14/12-6/22/12 for the Summer 2012 Semester.  This is the only time during the Summer 2012 semester that students can complete a Limited Enrollment Reinstatement, apply for Dependent Coverage, apply for permanent Exemption from the insurance fee, or apply for an Extension of your Spring 2012 coverage. Extensions are only for students not registering for the Summer Semester.  If you are in an exempt status (not covered under the plan), and you wish to remain exempt, you do not need to re-apply for exemption.  To check exempt status, look on your e-bill to see if you have been charged the UIUC Student Insurance fee (undergrad fee $219.00, graduate fee $310.00) If you have not been charged the fee, you are not covered by this plan.  Some students may not qualify to be assessed the fee.  
Necessary forms and instructions for exemption, extension, and dependent coverage can be printed off our web site: www.si.illinois.edu, under Important Forms, bottom of the page.  The deadline is 6/22/12. No applications will be accepted after the deadline.
